Immunization
With a strong emphasis on parent choice, we support your right to choose the best healthcare options for your individual family. We do ask that you help us to comply with the legal requirements placed on us as a public charter school program and promptly provide us with the completed immunization paperwork that reflects your choice. Options are detailed below. Immunization paperwork must be received by the first day of school, August 27 (Littleton) or 29 (Conifer), 2024.
Information about CIIS, the Colorado Immunization Information System referenced below. Vaccination and exemption data are submitted to CIIS by school districts, medical providers, and some online forms. You can choose to opt in or out of CIIS at any time.
There are four immunization records options for the 2024-25 school year
According to the CDPHE website, which is cited by the Jeffco Public Schools webpage on immunizations, “Colorado law requires all students attending Colorado schools… to be vaccinated against certain diseases unless they have a certificate of medical or nonmedical exemption on file.” There are four options to meet this requirement for the current school year. These options reflect new legislation that came into effect during the 2021-22 school year:
We often find immunization paperwork can be incomplete for the following reasons:
New immunizations or boosters are not completed according to the schedule followed by Colorado schools. The CDPHE website states, “The minimum number and spacing of doses is set forth in the Advisory Committee on Immunization Practices (ACIP) Child and Adolescent Immunization Schedules. Meeting the initial vaccine requirements does not excuse a student from meeting additional requirements.” If you do choose to submit vaccine records, they must be completed according to this schedule. Missing parent signature on an exemption form
Missing date on an exemption form (must be dated and submitted after July 1, 2024)
Failure to provide new paperwork, when required, for the current school year (see information under each of the four options above to determine whether new paperwork should be submitted each year).
Other Forms & Information
Health Plans
If your child has a reported health condition, a detailed health plan signed by your doctor must be turned in on or before orientation. All health plans must be updated for the new school year; these must be dated on or after July 1st and signed by your healthcare provider
Each year we need to collect a health plan for students with medical issues such as diabetes, allergies and anaphylaxis, asthma, and seizure conditions. Health plans must be turned in no later than the first day of school, August 27 (Littleton) or 29 (Conifer), 2024. Due to the legal requirements under which our program functions, students with medical issues are strictly unable to attend any classes at Summit Academy South without the appropriate health plan paperwork.
If you listed a medical issue on your student's information paperwork, you will need to contact your doctor and ask for a “health plan.” A health plan tells the school exactly what needs to be done to care for your student's medical needs. Doctors will be familiar with this request since it is required of any student in school. Some doctors have their own forms, and others request a form from us. Forms are available at: www.thesummitacademysouth.org/health.
Please note that trained school staff members must manage student health care while they are at school. We are strictly unable to allow student care to be managed by siblings or self (unless a medication falls under a self-carry exception and appropriate paperwork is on file).
Medication Paperwork
If you leave any medications in the office for your student to access during the school day, you must have a medication form completed and signed by your doctor for each medication. This includes over-the-counter medicines. Forms that allow students to carry EpiPens and inhalers are available, but no other medicines may be carried by students. All medication, including over-the-counter medicines, must be regulated and administered by trained staff.

Allergy Awareness
We ask that students and families be mindful that we do have students with food allergies on our campus. We need your help to keep the school setting as safe as possible.
Teachers may inform parents which foods cannot be brought into their classrooms. Please strictly follow all such restrictions, as they are to ensure the safety of students.
It is best if parents do not bring in food to celebrate student birthdays. Please consider other non-food items to celebrate (pencils, stickers, etc.).
Teachers are required to check rosters and use proper cleaning techniques before preparing food in class.
If your student has a food allergy, we will encourage them to follow these guidelines:
Eat only food that they have brought from home unless it is packaged, clearly labeled, and approved by their parents.
Wash hands before and after eating.
Do not share food, utensils, or containers.
